Office Policies

Co-payments must be made at the time of your appointment. We ask that all cancellations be made at least 24 hours prior to your appointment so that we can make room for other patients who need an appointment. You may be charged for missed appointments. Please arrive 10 to 15 minutes prior to your appointment if you are an established patient and 30 minutes early if you are a new patient. We request that you complete and sign all registration materials before your visit.

Prescription Policies
Please bring a current list of your medications or your medicine bottles to each appointment. All prescription refills should be made at the time of your visit. If you realize you will run out prior to an appointment, have the pharmacy contact us during normal business hours. Remember to allow at least 72 hours for the prescription to be filled. A new mail order prescription is best handled by you sending in the prescription we give you.

Insurance Information
Please bring ID and your insurance card with you to all appointments or you will be required to pay a portion of the visit prior to being seen. We will submit your claims to your insurance company. You will then be billed an amount not covered by your policy. Please pay this bill promptly so that service is not interrupted. We need a referral from your doctor, even if your insurance doesn’t require it. Those without insurance can choose to pay before leaving the office to receive a substantial discount.
